HEALTHCARE SAVINGS PROGRAMS (DMPO REQUIRED)
Some States require certain programs to be licensed as part of a DMPO. A DMPO is a regulated business entity that provides discounts on medical services. These discounts are usually available through contracts between the DMPO and various provider groups. DMPOs are generally regulated by state's Department of Insurance or Consumer Affairs. Currently, 13 states require specific DMPO licensing. The National Association of Insurance Commissioners has developed a model DMPO Act, and over the next few years, it is anticipated that a majority of states will adopt the model act or one similar.

Discount Medical Plan Organization
We offer Members access to savings of up to 25% and more through over 700,000 physician, medical facility, and hospital locations nationwide.
Members may access physician savings through the Point-of-Service Network of over 300,000 provider locations nationwide. The Point-of-Service Network offers the convenience of receiving your savings and paying the reduced amount at the time services are rendered.
Members who wish to have access to additional physician locations, or to facilities and hospitals, may utilize the Expanded Network of over 450,000 provider locations nationwide, plus over 100,000 medical facilities and over 70% of the hospitals nationwide. To access savings through the Expanded Network, Members are required to obtain a referral from the Plan Administrator and guarantee payment in advance of the visit with a major credit card (certified funds may be required for medical estimates exceeding $2,000).
To accommodate the needs of Member's who do not have a credit card or available credit, we have secured an Extended Payment Option which offers credit of up to $100 at 0% interest with repayment in three equal monthly payments.
In the event a Member faces hospitalization which will cost in excess of $5,800, and they are unable to guarantee payment, we offer assistance through a Patient Advocacy Program. The Patient Advocacy Program assists the Member and advocates on the Member's behalf with the provider to find an amicable solution for both parties.
The Medical Savings Program is the most comprehensive medical savings program available.
Point-of-Service Network
The Point-of-Service Network offers Members savings of up to 25% and more at the point-of-service at over 300,000 physician's locations nationwide.

Expanded Network
The Expanded Network provides Members access to savings through an additional 450,000 physician locations nationwide, plus over 100,000 medical facilities, and over 70% of the hospitals nationwide. All services through the Expanded Network must be pre-certified prior to the appointment and Members must guarantee payment in full to the Plan Administrator with a major credit card (or certified funds if services are in excess of $2,000). In the event a Member does not have a credit card, an Extended Payment Option may be available.
Extended Payment Options
We realize that not every Member may have a credit card or available credit. To accommodate these Member's needs, we have secured an extended payment option that is unmatched in the market. Credit up to $100 is available for physician visits. Repayment is required in three equal monthly payments, and best of all, there is 0% interest.
Patient Advocacy
Despite the significant savings offered, not all Members will be capable of raising the funds to pay for more extensive services. To assist these Members, we have partnered with an established Patient Advocacy Program to assist in evaluating financial options and negotiating with hospitals and physicians, based on multiple criteria, on claims exceeding, or estimated to exceed, $5,500.

Member Rights and Obligations AccessOne Consumer Health, Inc. a Discount Medical Plan Organization
The terms and conditions the ADMINISTRATOR (AccessOne Consumer Health, Inc.) Discount Medical Program Organization (DMPO) are outlined below.
Disclosures
- The plan is not a health insurance policy;
- The plan provides discounts at certain health care providers for medical services;
- The plan does not make payments directly to the providers of medical services;
- The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the DMPO. The corporate name and location of the licensed DMPO is :
- Access One Consumer Health. Inc., 809 East Main Street , Spartenburg, S.C. 29302
The DMPO will provide the member with a list of participating providers at its web site www.accessonedmpo.com or the member may call 1-800-896-1962 to find a provider. Members will be able to apply program discounts to all providers of each participating network. Provider services may include a combination of any of the following: medical doctors, doctors of osteopathy, dentists, chiropractors, diagnostic labs, out-patient facilities pharmacies, and ophthalmologist, optician, and optometrist services. The Member will be billed by the provider at the discounted rate at the time service is rendered. The Member is obligated to pay the provider for services rendered.
In no instance will the DMPO make payments directly to health care providers on behalf of the Member.
If the Member or the provider has a complaint regarding the DMPO then he or she go to www.accessonedmpo.com or call 1-800-896-1962 or write to Access One Consumer Health. Inc., 809 East Main Street , Spartenburg, S.C. 29302. This compliant will be addressed and the member will receive a response within 15 days of receipt of the complaint by the DMPO.
The Member is required to provide a list of all family members who will participate in this plan. If the member wishes to add or delete family members from this plan, the member must notify the DMPO at the contact information above at least 10 days prior to the effective date of any such change.
The member may terminate participation in the first 30 days of the program and receive a full refund on any fees or dues paid less a processing fee. After the first thirty (30) days, the member may cancel participation at any time. The Administrator must receive Notification at least (5) business days in advance of the next billing cycle for the member not to be charged for that billing cycle. If the notice of cancellation is not received prior to that billing cycle, then cancellation of payment will occur at the next billing cycle.
